Step 3 – Review & Sign Agreements

Signing Agreements

Signing all agreements is required.  These agreements are very important and define our working relationship.  They authorize us to setup your back-office.  They are also binding to Federal, State, and Local legal requirements for delivering health services online. 

Please review them carefully, they are:

  • Nutrition Factors Platform Terms-of-Use Agreement
  • Business Associate Agreement (BAA)
  • Provider Partner Commission Agreement
  • Referral Partner Program Agreement

If you have chosen not to participated in either a Commission Plan or the Referral Partner Program, your back-office will not be provisioned into these programs.  Later, when you choose to participate, you will be able to join the programs.

Click the corresponding buttons below to open and review each agreement.  A link to a PDF copy of each document is provided.  Click the links to open, save and/or print the agreements for your records.

To Complete Step 3

When you are ready to sign, complete the following on the form below:

  1. Click the check box for each agreement you are committing to.
  2. Type your full name in the box for each checked agreement.  (These are your digital signatures signifying agreement.  They are stored in Nutrition Factors’ back-office for your account.)
  3. Click Submit

That’s it!  You will be sent an email confirming your agreements.
